Is Your Disabled Child Transitioning
to Adulthood?
If you are the parent
of a child who is developmentally disabled and between the ages

that explain ''Transition Services".
Sponsored by Hennepin County's Human Services and Public Health
Department, the information sessions will outline decisions that
you will need to make such as guardianship, vocational goals, residential
options, Social Security and more.
To register, call 612-596-6631
or go to www.hennepin.us. click on "Health,
Housing and Social Services," "Developmental Disabilities" and "Contact
